Nespirah es una enorme semidiosa[1] (también conocida como una ancestro guardiana[2]), que se encuentra en la Silver Tide Trench frente a Glimmerdeep Gorge en la parte este de la Extensión Bruñida. Se encuentra en un estado de letargo, esclavizada por los naga. En su interior se consiguen poderosas perlas mágicas.
Earthmender Duarn y Erunak Stonespeaker siguen a los jugadores al interior del cuerpo de Nespirah. Erunak tiene cosas que hacer en el exterior y deja a Duarn al cargo. Duarn, con la ayuda de los jugadores, trata de comunicarse con Nespirah para obtener respuestas sobre el porqué ha permitido que los naga entren en su caparazón.
Es aquí donde los soldados de la Alianza y la Horda capturados han sido traidos desde el Bosque Kelp'thar. Un pequeño grupo de esclavos han escapado y luchan contra los naga sobre la plataforma situada sobre el corazón. Las actividades de los naga están lideradas por Overseer Idra'kess.
Nespirah Fluid fluye por el cuerpo de Nespirah y actúa como defensor contra invasores. Sin embargo, desde que la voluntad de Nespirah fue anulada por los naga, su fluído no es capaz de obedecerla. Todo esto cambiará cuando los naga dejen de tener influencia sobre ella y ésta despierte.
